New Patient Registration
The practice does not discriminate based on gender, race, sex, disability or age.
Anyone can register without needing proof of ID, address or immigration status.
We have a zero tolerance policy regarding abusive or violent behaviour towards our staff. Should a patient become abusive or violent then the patient will be removed from the Practice list immediately and an alternative Practice will have to be found.
You can register by collecting a paper copy from reception or completing the online registration form below.
Please ensure all non-optional parts of the form are completed. Registrations can take up to 14 to be completed and you will receive a text message once registered.
You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.
We recommend that new patients undertake a health check with a practice nurse.
Practice Boundary
We have an open list and welcome requests for registration from patients living in or moving to the practice area.
We accept patients from the following areas: DN1-1 DN1-2 DN1-3 DN2-4 DN2-5 DN2-6 DN3-3 DN9-3 (Auckley & Finningley) DN4-5 DN4-6 DN4-7.
If your home address is not included in one of the postcodes above, you are still welcome to register with the practice. You will however, be registered as a out of area patient. All this means is that the practice may not be able to offer you a home visit should you require one.
Non-urgent advice: Patient Notice
The Practice is a 2-centre site and patients can be expected to attend appointments at either surgery – especially is requesting an urgent same day appointment.
